We would like to inform you of an exciting new program that we are offering at Shade Tree Assisted Living in Brownton, MN serviced by Central Minnesota Senior Care. We are revamping our program to specialize in adults 55 and older with Mental Health needs. We now have an onsite LPN with mental health and activity experience along with around the clock care by trained ULP’s. The goal of this program is to provide housing for adults 55 and older with a Mental Illness Diagnosis in a comfortable assisted living as an alternative to a nursing home. We will have activities and programming suited to fit their individual needs.
Our Program Coordinator will ensure the Client’s need are being met by promoting and encouraging independence, activities, socialization and community outreach. We will coordinate care with the Client, Family, Physician, Therapists, Director, RN, Social Worker, Direct Care staff and any other providers the Client requires.
